We want to ensure that our patients across the Axe Valley have the best possible health outcomes. In order to achieve this our PCN team works closely with the GP practices to support their patient populations to access the most appropriate care – whether this is at home or in a care home.
The practices and PCN work closely together to understand our patients’ needs and ensure that they are provided with care either directly by ourselves or in collaboration with local partners; local organisations, health and social care, community groups and the voluntary sector.
