What is a PCN?

A Primary Care Network (PCN) is a group of GP practices working together with other healthcare and social care providers to deliver integrated services to a defined local populationTASC PCN (Townsend, Axminster, Seaton & Colyton) serves around 30,000 patients in the Axminster and Seaton area and is designed to improve patient care by coordinating services and sharing resources. 

PCNs are built around the needs of a specific geographic area and its population and are able to provide focused local resources in both the GP surgeries and local area. PCNs enable close collaboration between GP practices themselves as well as enabling a closer working relationship between the GP surgeries and other local providers of care.

TASC PCN works closely with the local practices to reduce duplication of work, share resources and learning and provide an effective and safe delivery of services.
