Pharmacy Team

In a PCN, pharmacists and pharmacy technicians play crucial roles in supporting patient care, enhancing clinical outcomes and improving the efficiency of healthcare services.  Our clinical pharmacist is an independent prescriber works with our GPs to undertake medication reviews and to provide clinical support around hypertension.  Our pharmacist can provide advice on medication use, side effects and lifestyle changes, enhancing patient engagement and self-management.

We work closely with the Devon Medicines Optimisation Team to ensure that we are identifying and reducing unnecessary or duplicate medications, contributing to better use of NHS resources and take part in projects on specific medication types.

Our pharmacy technicians manage safety alerts to ensure that these are communicated to the relevant teams within the practices.  They also work closely with our local hospitals to implement recommendations on discharge summaries when a patient has had a stay in a secondary care environment.  They help practices to comply with local formularies, contributing to cost control and supply efficiency.  In order to ensure that we support our patients to understand their medication and provide assistance where needed our pharmacy technicians will provide direct assistance and help resolve minor medication- related queries.

The TASC PCN pharmacy teams works collaboratively with GPs and other healthcare professionals to enhance patient care, optimise medication use and support the overall goal of integrated, person-centred healthcare delivery.
