Social prescribing link workers

Our social prescribing team support patients with long-term conditions, low level mental health issues and those who feel lonely or isolated. They work with GPs to provide a health pathway for those people who would benefit from non-clinical support.

Part of the role is to connect patients to groups, activities and services in the local community to meet their social, emotional and practical needs that might be affecting their health and wellbeing.

We also provide one to one health and wellbeing sessions with our trained coach to patients needing support to make positive lifestyle changes.  This would focus on behaviour change and self-management.

Generally, patients would receive an initial 6-8 sessions with one of the team after which they might be referred for further support or discharged.

Patients should contact their surgery through an online consultation or by telephone in order to be referred to our service.
